Total hip replacement
Total hip replacement (total hip arthroplasty) is a well-established surgical procedure in which the arthritic surfaces of the hip joint are replaced with prosthetic components. It is commonly used to reduce pain and improve function in people with end-stage hip arthritis.
Causes of hip arthritis
- Primary osteoarthritis — the most common cause, related to wear and degeneration over time
- Rheumatoid and inflammatory arthritis
- Post-traumatic arthritis following fractures or dislocations
- Avascular necrosis — loss of blood supply to the femoral head
- Hip dysplasia — abnormal joint development
Symptoms
- Groin, thigh or buttock pain
- Stiffness — particularly difficulty putting on shoes and socks
- Pain that limits walking distance
- Night pain disturbing sleep
- A limp
The procedure
Under general or spinal anaesthesia, the damaged femoral head (ball) is removed and the acetabulum (socket) is resurfaced with a metal cup and plastic or ceramic liner. A metal stem is placed in the femoral canal, topped with a new femoral head component. Components may be fixed with bone cement or press-fit in place.
Dr Myhill will discuss bearing surface options, fixation method and surgical approach with you at your consultation.
Recovery
Walking typically begins on the day of or the day after surgery. Hospital stay is typically 2–4 nights. A physiotherapy programme continues for several months. People typically return to light activities within about 6 weeks and to most normal activities by around 3 months, but recovery varies between people.
Risks and complications
All surgery carries risk. Complications can include:
- infection
- blood clots in the legs or lungs
- nerve or blood vessel injury
- dislocation of the new hip, or a difference in leg length
- implant wear or loosening, which may mean further surgery in future
- risks related to the anaesthetic
Dr Myhill will talk through the risks that apply to you at your consultation.
